Your clinic assigns a journey
A journey is built for one condition. It sets the questions you are asked, the sessions you work through, the readings that matter and how long it runs.
You check in each day
A short set of questions, the same time each day, usually under a minute. If you would rather not use the app, someone can call and record it with you.
Today's plan is already there
Your check-in, the session that has unlocked, anything outstanding. You are never deciding what to do next.
Progress builds up
Streaks, momentum over the last fourteen days, trends in your readings, and milestones as you pass them.
A person picks things up
When something looks like it needs attention, it reaches your care team as a task with a reason attached, not a notification into a void.
What a check-in actually asks
It depends on the journey. A blood pressure journey asks about readings, medication and symptoms. A nutrition-led journey asks about food and routine. The clinic configures the questions; nothing is generic.
You can skip a question, say you do not know, or say it was not available. Those are recorded as distinct answers rather than being treated as a blank, because "I did not measure" and "it was normal" are not the same thing.
